This activity will help you determine which resources or research will be most relevant to address a particular need. This may be useful as you consider how to explain the purpose and relevance of the resources you are assembling for your tool kit. The activity is for your own practice and self-assessment, and demonstrates course engagement.

PROFESSIONAL CONTEXT

Nurses are often asked to implement processes, concepts, or practices – sometimes with little preparatory communication or education. One way to encourage sustainability of quality and process improvements is to assemble an accessible, user-friendly tool kit for knowledge and process documentation. Creating a resource repository or tool kit is also an excellent way to follow up an educational or in-service session, as it can help to reinforce attendees’ new knowledge as well as the understanding of its value. By practicing creating a simple online tool kit, you can develop valuable technology skills to improve your competence and efficacy. This technology is easy to use, and resources are available to guide you.

SCENARIO

For this assessment, consider taking one of these two approaches:

  1. Build on the work done in your first three assessments and create an online tool kit or resource repository that will help the audience of your in-service understand the research behind your safety improvement plan pertaining to medication administration and put the plan into action.
  2. Locate a safety improvement plan (your current organization, the Institution for Healthcare Improvement, or a publicly available safety improvement initiative) pertaining to medication administration and create an online tool kit or resource repository that will help an audience understand the research behind the safety improvement plan and how to put the plan into action.

PREPARATION

Google Sites is recommended for this assessment – the tools are free to use and should offer you a blend of flexibility and simplicity as you create your online tool kit. Please note that this requires a Google account; use your Gmail or GoogleDocs or create an account following the directions under the “Create Account” menu.

INSTRUCTIONS

Using Google Sites, assemble an online resource tool kit containing at least 12 annotated resources that you consider critical to the success of your safety improvement initiative. These resources should enable nurses and others to implement and maintain the safety improvement you have developed.

It is recommended that you focus on the 3 or 4 most critical categories or themes with respect to your safety improvement initiative pertaining to medication administration. For example, for an ;initiative that concerns improving workplace safety for practitioners, you might choose broad themes such as general organizational safety and quality best practices; environmental safety and quality risks; individual strategies to improve personal and team safety; and process best practices for reporting and improving environmental safety issues.

Following the recommended scheme, you would collect 3 resources on average for each of the 4 categories focusing on safety with medication administration. Each resource listing should include ;the following:

  • An APA-formatted citation of the resource with a working link.
  • A description of the information, skills, or tools provided by the resource.
  • A brief explanation of how the resource can help nurses better understand or implement the safety improvement initiative pertaining to medication administration.
  • A description of how nurses can use this resource and when its use may be appropriate.

According to (The Future of Nursing, 2011) in order to provider great quality of care we must first work on ensure that nurses can practice to the full extent of their education and training, improving nursing education, provide opportunities for nurses to assume leadership positions and to serve as full partners in health care redesign and improvement efforts and improve data collection for workforce planning and policy making. According to (Bridges,Davidson, Odegard, Maki, & Tomkowiak, 2011) an Interprofessional collaborative practice consists of communication and decision-making, enabling a shared combination of different discipline and social expert knowledge and skills to derive at a process or goal to resolve or close gaps in care.

Interprofessional collaboration is needed in health care due to the different complex conditions patients are presenting. This team approach ensures all areas of patient care was discussed by all discipline and expert recommendations are being utilized. According to (Janssen, Sagasser, Laro, E., de Graaf, & Scherpbier-de Haan, 2017) intraprofessional collaboration is expert decisions being made by the same discipline. These decisions are not based on other disciplines, just the expert outcomes of one discipline; it is a review of the core competencies for that discipline and the patient is always incorporated. . (2011d), obtaining a Doctorate in Nursing (DNP) will provide Advance Practice Nurses with the education, tools, resources and expertise to lead the profession, collaborate with other disciplines to improve patient care and also teach them about the profession and also embarrass other professions.

I am currently working in an environment where I have to collaborate with other healthcare professionals including the patient, family or significant others in order to help my patients achieve their goals for coming to the rehabilitation hospital. This is what we do inside the Inpatient Rehabilitation Facility ( IRF ). Our patients must be able to tolerate and benefit from 15 hours of therapy weekly. The type of conditions we currently treat are stroke , spinal cord injury , congenital deformity , amputation , MVA , brain injury , Parkinson’s disease, arthritis and patients that recently undergone hip and joint surgery. In order for these patients to succeed in their rehabilitation program they need a good team of healthcare workers. “ Rehabilitation , as a specialty , has one of the longest histories of using teamwork to help meet client goals , but working as a member of a team is not always easy ” ( Vaughn , 2019 , p.29 ).

Dr. Joan Stanley explained why by saying “ And interprofessional communication is critical, and in order to lead teams, to work within systems, to make change, advanced practice nurses need to be able to communicate and understand, and it’s not just communicating, but it’s also understanding the other professionals’ perspective, knowing where they come from, understanding the background, and also being able to explain to others the nursing perspective “ . “Success or failure of the team is dependent on the effectiveness of the communication process” (Zaccaginini & Pechacek, 2021, p.179). As a DNP prepared nurse my responsibility towards my patients is broader.

Dr Susan Stefan says “ As a nurse with a doctorate, we have the opportunity to be at the same level as other professionals. As a staff nurse, even though we probably know more about the care that patients need on some levels and certainly we’re in a role of coordinating patient care, we’re just not at that same level, and the DNP helps bring nurses to that level where our opinions and our skills are more recognized “. I think the multidisciplinary team where I work welcomes my ideas and opinions with an open arms as i already established a good working relationship with them. My place of work is the perfect environment to put my learning into practice.

In our hospital we are required by The Centers for Medicare & Medicaid Services ( CMS ) to have team meeting three times in a week to discuss about our patients condition , their progress and barriers for discharge. As a member of the team I have to make sure that the leadership on the team based upon patient needs. The other way I can share my knowledge I gained as a DNP prepared nurse is through our daily nursing huddle or during our Town Hall meeting with the CEO. At the moment I am not in the position of leading this meeting but I can set myself as a good example by participating in the nursing huddle using an SBAR. Many successful teams utilize the SBAR process as a common language means to convey critical information to be shared. It provides clear expectations as for what and how information is to be communicated among the team members and by its use promotes patient safety ( Leonard et al., 2004 ; RWJF , 2015 ).

During the town hall ,meeting one of the most important agenda that our CEO would discuss to us are the patient outcome and patient experience. The DNP prepared nurse can play a role by joining with the team in sharing their concerns, offering ideas in solving a problem and be part of the discussion. To improve quality, maximize resources, and coordinate care , patents with complex health needs are best served by an interdisciplinary approach to care. To foster collaborative behaviours between professionals, reduce biases of other disciplines, and ultimately improve patient outcomes, nursing education needs to expand to include competencies in interprofessional teamwork and collaboration in clinical environment (IOM,2001).

The mission, vision and values of the organization is to achieve service excellence, and provide the safest, high-quality healthcare to the community which it serves, and vows to work collaboratively with the community and staff to ensure that innovation, technology and best practice is always utilized (The Valley Hospital, 2019). The formal structure of the organization is hierarchical. According to Hickey & Brosnan (2017), hierarchical organization structures have many layers, similar to a pyramid, where communication flows from the top down or from the bottom up and each layer is subordinate except for the top tier.
 
Although the structure of the organization is hierarchical, the leadership style and culture of the organization more bureaucratic where decision-making is delegated and shared amongst leaders and staff at various layers throughout the organization (Hickey & Brosnan, 2017). The culture or informal structure of the organization is strong and positive toward the community, its staff and safety. The healthcare system has embarked on a high-reliability journey and is fully committed to transparency, accountability and safety. As a result, the work environment has improved, however, is still affected by staff turnover and retention. Overall, the healthcare system has a strong reputation in the community, through social media, as well as Leapfrog and Healthgrades. The hospital has been Magnet designated since 2009, which has provided a strong environment for nurses to thrive and be a vital element of research, innovation, evidenced-based practices and collaboration on ways to enhance and improve patient care delivery.
 
Frameworks & Improvement Opportunities
In the article by Sadeghi, S., Barzi, A., Mikhail, O., & Shabot, M. M. (2013), the authors discuss the framework of a patient-centered perspective to healthcare. Within this writers’ healthcare organization, this framework was applied to developing care standards for the LGBTQ community. Throughout the healthcare system, patient-family centered care advisors are utilized. Some are employed and others are volunteer. From developing stroke pathways in the Emergency department to creating scripts for discharge follow up nurses; our patients are members of many of our hospital-based committees. This framework has allowed the healthcare system to understand the essence to best serve each patient and execute service excellence. Another framework is the concept of good to great. The good to great method is taking a complex system and streamlining the consistent drive to adhere to an organizations overall mission (Gellar, 2006).
 
Each pillar of the good to great concept is already reflected throughout this writers’ healthcare system as it is striving for zero harm, 100% reliability, and a strong culture of safety and transparency. However, an area of opportunity to improve is utilizing employee talents. As a Magnet designated organization, an overwhelming number of nurses are bachelor, masters, and doctorate level prepared. The organization does not always utilize the staff as internal experts. An example is hiring a company to lecture on evidenced-based practice when the hospital has full-time PhD educated nurses fully capable of doing this. The same goes for Advanced Practice Nurses (APN). The hospital does not adequately utilize APN’s for patient care, rather they take on an administrative role with quality initiatives such as Medication Reconciliation, or are database administrators. This does not correlate with the organizations overall mission, vision and values to grow and utilize staff talents to build to produce high quality, safe care.

As leaders in nursing, it is imperative to be able to identify and understand ones’ organizational structure. The formal structure is important as it reflects the formal positions within an organization as well as the lines of communication. The informal structure refers to the culture of an organization and may be more difficult to identify. Being in tuned to ones’ surroundings and observing how communication flows and positions held, can yield great insight into informal structures. By having an understanding of a healthcare organizations structure, leaders can be more effective which provides the best opportunity to achieving shared goals and success.

As nursing leaders, it is essential to understand both the formal and informal structures within an organization. Formal structures are easily retrievable and describe the hierarchy of positions and chain of command in an organization (Laureate, 2013f). On the other hand, informal structure refers to the power, culture, and politics within an organization, which is harder to identify (Laureate, 2013f). The purpose of this discussion is to describe an analysis of the systems and structures of ones’ healthcare organization, explain insights gained by comparing two frameworks, and how each are used to assess an organization, identify a need for improvement, and enhance the performance of an organization.
